“Become involved. Everyone has something to contribute.”
If your child has to take medication while at school, be it prescription or over-the-counter, you will need to complete this form for each medicine, including the prescribing health care provider’s signature and directions from that health care provider for proper administration, and return the completed form to the school nurse.
The medication, whether prescription or non-prescription, must be in its original labeled container. If you were given “samples” of any medications, they must also be in a container that appropriately identifies the medication.
If you have questions you may contact the School Nurse at 821-3458 or the District Office at 821-1000.
